Area schools set registration dates

Buncombe Grade School 

Buncombe Grade School will have registration for the 2014-2015 school year on Tuesday, August 12, from 9 a.m. until 3 p.m. Proof of residency, such as a tax receipt, utility bill or other official proof will be expected at the time of registration.

All students must be fully immunized according to the requirements of the Illinois Department of Public Health.

• All students must have the Hepatitis B inoculation completed prior to the first day of school.

• Kindergarten, sixth grade and transfer students must have a completed physical examination on file with the district prior to the first day of school.

• Kindergarten, second grade and sixth grade students need proof of dental exams.

• Kindergarten students must have proof of vision examinations.

• Student athletes must have a physical on file prior to the first day of practice.

• Any student with any diagnosed acute or chronic illness will be required to have a completed health care plan on file with the district.

New Simpson Hill 

New Simpson Hill will have registration for the 2014-2015 school year on Thursday, July 31, from 9:00 a.m. to 6:00 p.m.

NSH will open the school year with Staff Institute Days on Tuesday, August 12.

The first day for student attendance will be Wednesday, August 13, with a 2:15 p.m. dismissal.  School will be dismissed daily August 14-15 at 2:15 p.m.

The regular schedule will begin Monday, August 18 with dismissal at 3:00 p.m.  Bus routes and times will be about the same as last year.

All students must be fully immunized according to the requirements of the Illinois Department of Public Health.  All students (PK – 8) must have the Hepatitis-B inoculation completed prior to the first day of school.  Kindergarten, 6th grade, and transfer students must have a completed physical examination filed with the district prior to the first day of school.  In accordance with legislation, proof of dental exams will be required for students in grades K, 2, and 6.  Vision examinations will be required for kindergarten students.  Student athletes must have a physical filed prior to the first day of sports practice.  Student insurance information will be available at registration.

Fees are due on registration day and are based on the following:

Breakfast – $1.00

Cap & Gown

8th Grade – $25.00

Reduced Breakfast – 30¢

Lockers – $2.00

Lunch – $1.60

Registration & Book

Rent – $25.00

Reduced Lunch – 40¢

Milk – 30¢

Other information required on registration day includes: phone number (home, work, cell); information for 3 people to contact in case of emergency; doctor’s name and number; Medicaid number if applicable; copy of certified birth certificate; and a Social Security number.  For new students, transfer students and students with a change of address, three forms of proof of residency will be required.

Calendars, menus, and other notices of interest to parents will be sent home each month that school is in session.

Breakfast and lunch will be served daily beginning on Wednesday, August 13.

Cypress Grade School

Cypress Elementary School will hold student registration days August 5 from 3 p.m. – 7 p.m. and August 6 from 8 a.m. – noon.

The first day of student attendance will be August 13th with a 1:30 p.m. dismissal time. School will dismiss at 1:30 p.m. August 13 – August 15. The regular schedule will begin Monday, August 18.

All students must have current immunizations on file. Kindergarten and 6th grade students must have physical examinations on file before the first day of school. Dental examinations must be on file for Kindergarten, 2nd grade, and sixth grade students. Kindergarten students must also have vision exams. Student athletes are required to  have a current physical on file prior to the first day of practice.

Information required for registration includes address, phone number, emergency contact information.  If you are unable to register during these times, please call the school at 618-657-2525 to schedule a registration time.

Vienna Grade School

Vienna Grade School begins the 2014-2015 school-year on Wednesday, August 13. School will be dismissed at 2:00 p.m. on the first day, with a 2:00 p.m. dismissal on August 14th and 15th.

Bus routes and times may vary for the 2014-2015 school year and more information will be available at registration.  Please call the school office at 658-8286, if you need information regarding transportation.

All students must register for the 2014-2015 school year on July 30 or July 31, 2014.  Please enter the school on Third Street, Door B, for registration.

Due to auditing procedures by the State Board of Education, parents should be prepared to present documentation that certifies their correct 911 address.  A tax receipt, utility bill, or other official proof of residency may be used.

Requirements for school entrance by grade:  Pre-kindergarten (prior to entering for the first time) physical examination, lead screening documented, eye examination, up-to-date immunization record, and chickenpox vaccine required or documentation of disease.  Dental examination recommended, but not required.

Kindergarten requirements are physical examination, dental examination, lead screening, eye examination, up-to-date immunization, and chickenpox vaccine required or documentation of disease.

Second grade requirement is a dental examination.

Sixth grade requirements are physical examination, up-to-date immunization record, and dental examination.

In addition, the following are required: completed health care plan for any diagnosed acute or chronic illness.

Eye examination for any out-of-state transfer student entering an Illinois school for the first time is also required.

School medication authorization form completed and signed by a health care provider along with parental/guardian consent.

Any student who participates in a sport or an extra-curricular program that requires a health examination will still need to provide proof of current physical in order to participate prior to the first formal school practice for any respective sport season (sports physicals are valid for 395 days).

Past due balances must be paid at or before registration.

Book rental fees must be paid at registration in order to register.  Lunch and breakfast money must also be paid at this time.

Fees are as follows:

• Book rental fee, K-8 – $15.00

• Instructional Technology Fee – $15.00

• 8th Grade Graduation Fee – $25.00

• Lunches – $1.95 per day

• Breakfast – $1.25 per day

•Kindergarten Snacks – $25.00 per year

• Milk – 30¢

NOTE – You must have the following information to register your child:

1. Child’s name

2. Child’s date of birth (Certified birth certificate must be on file)

3. Child’s Social Security number

4. 911 address and a mailing address

5. Telephone numbers: home/work/cell

6. Physician’s name and telephone number

7. Three emergency contacts including name, telephone number and relationship to student.

This information is important to us, so please bring this information with you on registration day.

Vienna High School

Registration for the 2014-2015 school year will be held in the Vienna High School’s Commons Wednesday, July 30, from noon until 6:00 p.m. and Thursday, July 31, from 7:00 a.m. until noon.  A parent or guardian must accompany the student on one of these days to complete the registration process.

The first day of student attendance will be Wednesday, August 13, with a 2:00 p.m. dismissal. There will be a 2:00 p.m. dismissal schedule August 14 – 15.

Physical, Immunizations and Dental Examinations

Students entering the 9th grade, transferring students, or students participating in athletics must provide a copy of a current (within year) physical and immunization records in order to register or participate in a school sport. State law requires diabetes screening to be completed on all school physicals (K, 1, 5, 9, Pre-K, or other programs offered through the school). An eye exam is required for all students entering the public school system or the Illinois public school system for the first time. A dental examination is also required for incoming ninth grade students or students new to the district.

At registration, it is required that all parents provide the school with an emergency phone number where they may be reached during the day.

The Department of Public Health shall establish a waiver for children who show an undue burden or lack of access to a dentist.  Each public, private, and parochial school must give notice of this dental examination requirement to the parents and guardians of students at least 60 days before May 15 of each school year.

Photographers, ID Cards, Senior Pictures

Photographers from Lifetouch will be available after the start of school for the yearbook and student identification cards.  There is no charge for these pictures. All seniors are required to get ID photos taken at no charge.  Substitute photos (3 copies) may be given to the yearbook sponsor by February 1 or the Lifetouch ID photo will be used in the yearbook.  A senior picture must also be submitted to the superintendent’s office by February 1st for the Vienna Times senior class issue.  If a student wishes to purchase a picture packet, flyers with prices will be available at this time.

Students must carry their plastic student ID card every day.  The first ID card is free, subsequent replacement cards are $5.00 each.

Bus Route

Bus route will follow the same patterns as last year. If you are new to the District, please call or stop by the high school so we can determine which bus route and estimated time of pick-up.

Breakfast and Lunch

All parents are asked to complete a free and reduced lunch application during registration.  Our goal is to provide all eligible students with free or reduced breakfast and lunch.  Students, who qualify, are also eligible for the $40 book and locker rental fee waiver.

Money may be placed on account at registration or the first day of school for lunch and breakfast.  Lunch is $2.25 and breakfast is $1.50.  Student’s identification cards are scanned to deduct money from their account.

Books and Lockers

Book and locker rental fees will be $40.00.  All students must purchase P.E. uniforms, if they do not have one from a previous year.  Physical education uniforms will be approximately $16.00.  Optional student accident insurance can be purchased.  Parking stickers will be $3.00 for those who drive automobiles on campus.  You will need to provide information on the make, model, color, and license number of the vehicle you are registering.

Drivers Education

There will be a $50.00 class fee for driver education during the school year for all eligible students, payable to Vienna High School on registration day.  This fee must be paid when the student registers for class.  At a later date a $20.00 fee payable to the Secretary of State will be required to take the learner’s permit test.  State law requires students to maintain a minimum grade point average to be eligible to get a learner’s permit.

Residency

Prior to attending classes, a completed registration form and affidavit or residency must be on file for each student.  The school administrator or designee will require at least two documents that prove residency in the district’s boundaries.  Those documents may include the following: property tax receipt for the residence; current utility bills; current rent receipt, including address of property, name and address of owner, date and amount; driver’s license with 911 address; Public Aid Card, including child’s name, assigned # and caseworker; or voter’s registration card with address. Copies of these documents will remain in the student’s temporary file and are required prior to school attendance.  Students who are not residents of the district may attend Vienna High School upon payment per capita tuition.  For further information please see Vienna High School District 13-3 2014-2015 Student-Parent Handbook.

Providing false information to a school district regarding the residency of a student for the purpose of enabling that student to attend school without the payment of nonresident tuition is a Class C misdemeanor.

Students, parents, and patrons need to be aware that surveillance cameras are being utilized on the premises of Vienna High School.
